How to complete the
Claim Registration Form
Complete as much of the form as you can.
It's okay if you do not know all the answers.

Start with you full legal name and date of birth.
Add any other information you know, such as your name while in foster care, contact information, CFS Agency, or approximate years in care.
If someone is completing the form for you, they should also complete the Representative Information section.
If you have ID or supporting documents, include copies. If you do not have ID, you can still start your claim.
Before you submit, review the Acknowledgement, Authorization and Declaration, sign and date the form, and make sure your contact information is current.
You do not need to have everything ready
You do not need to remember your exact dates in care.
You do not need to know the name of your CFS Agency.
You do not need to have all of your documents before starting.

Using a Guarantor
If you do not have current government-issued ID, a trusted service provider may be able to help confirm your identity.
When to use a guarantor
You do not have current government-issued ID.
Your ID has been lost of stolen.
You have difficulty getting replacement ID.
A trusted service provider knows you and can help confirm your identity.
Who can be a guarantor?
A support worker, case worker, social worker, community worker, or another approved service worker who knows you.
How to use the guarantor form
Start with your Claim Registration Form.
Ask an appropriate service provider to complete the guarantor form.
The guarantor provides their name, organization, role, contact information, and how they know you.
The guarantor confirms your identity based on personal knowledge, organization records, or both.
Submit the completed guarantor form with your Claim Registration Form, or have the guarantor send the completed form seperatly.
Don't have ID or someone who can act as a guarantor?
That's okay.
