How to Submit a Claim

Submit your Claim Registration Form and supporting documents by January 5, 2027 to Exchange Solutions Inc. using any one of the following:

Online Submission:  Complete your Claim Registration Form and supporting documents through the Online Submission.

Download:  Complete your Claim Registration Form and submit in the following ways:

Claimant Upload Portal:  Upload your completed Claim Registration Form and supporting documents.

Claim Registration Form Instructions:  Explains the information requested in the Claim Registration Form.

Declaration of Guarantor for Proof of Indentity:  If a Claimant (Child-in-Care) does not have ID, please arrange for this form to be completed and submitted with the Claim Registration Form.

 

Please make sure to keep a copy of your Claim Registration Form package and any attached documents for your personal records.

 

Do not send in original documents – clear photocopies or pictures will be accepted.  The Claims Administrator cannot guarantee that any original documents sent will be returned.
